Southeastern Rural Cancer Care Network

Cancer patient navigation is an evidence-based intervention that leads to better cancer outcomes. Although many large, urban hospital systems employ navigators, many rural health care systems and their patients are left without this vital resource.

Stepping up the fight in SE Georgia

Georgia CORE capitalized on the robust relationship among health providers in southeastern Georgia and its own strong research collaboration with St. Joseph’s/Candler to build the Southeastern Rural Cancer Care Network. The effort is funded by a four-year grant from the Health Resources & Services Administration.

What’s offered?

The network provides three primary services to improve the quality of cancer care in rural Georgia. With a multi-disciplinary approach, including the hiring of a regional oncology patient navigator, the Southeastern Rural Cancer Care Network provides:

  • Patient navigation services and coordinated care
  • Patient access and enrollment to clinical trials
  • A process for developing and implementing sustainable strategies of care that may be replicated in other under-resourced areas of the state

Who’s in the network?

  • Appling Healthcare Hospital (Baxley)
  • Wayne Memorial Hospital (Jesup)
  • The Nancy N. and J.C. Lewis Cancer & Research Pavilion at Wayne Memorial (Jesup) and at St. Joseph's/Candler (Savannah)

Which areas are served?

While network partners are located in Wayne and Appling Counties, the network also covers nine surrounding counties: Atkinson, Bacon, Brantley, Coffee, Jeff Davis, Pierce, Toombs, Tattnall, and Ware. Serving all 11 counties is important to address the barriers to care and disparities across areas that lack oncology services.
